In 2018, Western Australia experienced a spike in homicides allegedly associated with individuals who were patients of, or had recent contact with public sector mental health services. This preliminary Review of the Root Cause Analyses (RCAs) and other documentation completed by the Health Service Providers (HSPs) has been initiated by the Chief Psychiatrist as a means of informing him on whether there is a need for a broader review process in an attempt to clarify, if possible, what factors may be associated with this spike in homicides by persons with a history of mental illness.
